The Benefit & Wellness Program is only valid at Abbett & Associates and is not an insurance plan. It cannot be used in conjunction with any form of insurance plan or discount program and is secondary to any other dental plan.
Program fees are non-refundable, including services that are underutilized. Failure to cancel 48 hours prior to appointment will forfeit the value of planned services. Patient’s portion of bill is due at service date.
This benefit program is not valid towards hospital charges of any kind, nor is it valid towards referrals to specialists. This program cannot be used for treatment in which the provider feels is outside the realm of standard dentistry.
